Medical Services

Moonshot Medical and Performance provides medical optimization, hormone therapy, body composition analysis, and related health services. By engaging our services, you acknowledge:

• Our services are not a substitute for emergency medical care. If you are experiencing a medical emergency, call 911.
• Treatment plans are individualized based on your health history, lab results, and clinical evaluation.
• Results vary by individual. We do not guarantee specific outcomes from any treatment or protocol.
• You are responsible for providing accurate and complete health information.

Appointments & Payments

• Appointments must be cancelled or rescheduled at least 24 hours in advance. Late cancellations or no-shows may be subject to a fee.
• Payment is due at the time of service unless otherwise arranged.
• We are a cash-pay clinic. We do not bill insurance for medical optimization services.
• HSA and FSA cards are accepted for eligible services.

Membership Programs

If you enroll in one of our membership programs:

• Membership fees are billed monthly and are non-refundable once processed.
• You may cancel your membership at any time with 30 days written notice.
• Membership benefits are for the enrolled individual only and are non-transferable.
• Included services (labs, DEXA scans, etc.) must be used within the membership period and do not roll over.

Website Use

When using our website, you agree to:

• Use the site for lawful purposes only.
• Not attempt to gain unauthorized access to any portion of the site or its systems.
• Not reproduce, distribute, or create derivative works from our content without permission.

Limitation of Liability

To the fullest extent permitted by law, Moonshot Medical and Performance shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services or website. Our total liability shall not exceed the amount paid by you for services in the twelve months preceding any claim.

Changes to Terms

We reserve the right to modify these Terms of Service at any time. Changes will be effective immediately upon posting to this page. Your continued use of our services or website after any changes constitutes acceptance of the new terms.

Governing Law

These Terms of Service shall be governed by and construed in accordance with the laws of the State of Illinois, without regard to its conflict of law provisions.
