"Weight loss peptide" is a marketing phrase the internet uses to mean different things. In legitimate clinical practice, the weight-loss peptides patients are actually losing weight on are the GLP-1 family — semaglutide and tirzepatide. They are peptide-based incretin mimetics. They work. The trials are clear, and so is the real-world evidence.

The pattern I see most often is patients who started one of these through a telehealth-only company without baseline labs, without body composition tracking, and without anyone monitoring whether they were losing fat or losing muscle. They drop 30 pounds, feel weaker, get worse cardiovascular markers in some cases, and lose bone density — especially the women in perimenopause who are already at risk.

Real measurement at the start. Real measurement during. Real follow-up. That's what makes a weight-loss peptide a weight-loss medicine.

THE WEIGHT-LOSS PEPTIDE FAMILY

The peptides that actually drive weight loss in clinical practice are GLP-1 receptor agonists. Each has a different profile.

Semaglutide

Single-receptor GLP-1 agonist. Phase 3 trial average ~15% body weight loss at 68 weeks. Brand names Ozempic and Wegovy. Available compounded through licensed 503A/503B pharmacies. Longer real-world track record than newer agents.

Tirzepatide

Dual GLP-1 + GIP receptor agonist. Tends to produce greater average weight loss in head-to-head trials (~20–22% at high dose). Brand names Mounjaro and Zepbound. Different tolerability profile — some patients tolerate it better than semaglutide, some worse.

What about the next-generation peptides?

Newer triple-agonist GLP-1/GIP/glucagon peptides are in trials but are not currently FDA-approved and are not eligible for compounding. We follow the data and update our formulary as the regulatory and clinical landscape changes.

Other "weight loss peptides" online

A lot of marketing labels other peptides as "weight loss peptides" — most of these have no meaningful clinical evidence for fat loss in humans. We focus on what the trials and the real-world data actually support.

COMMON QUESTIONS

Are GLP-1 medications actually peptides?

Yes. Semaglutide and tirzepatide are peptide-based molecules — chains of amino acids modified for stability and receptor binding. They are the most clinically validated weight-loss peptides currently available.

Semaglutide vs tirzepatide — which is better?

Tirzepatide tends to produce greater average weight loss; semaglutide has a longer real-world track record. Tolerability differs by patient. We discuss both at intake. Read the comparison.

Will I regain the weight if I stop?

Many patients regain partially. Sustainability depends on what your habits and body composition look like when you discontinue. Patients who built lean mass during treatment and learned a sustainable eating pattern hold weight loss better than those who only lost weight.

What about other "weight loss peptides" I see online?

A lot of products labeled as "weight loss peptides" online have no clinical evidence behind them for fat loss in humans. We prescribe what the trials and real-world data support — currently the GLP-1 family. If a new molecule earns the evidence, we'll evaluate it.

Do you take insurance?

No. We're a self-pay clinic. If your insurance covers brand-name Wegovy, Ozempic, Mounjaro, or Zepbound, we can write the prescription and you fill it externally — but our program (compounded medication + visits + DEXA) is not billed through insurance. HSA/FSA accepted.
